Client Contracts and Forms

Forms are typically completed via TherapyNotes Electronic Health Records Portal. If you are unable to complete them on the portal, Download the forms here. Please return them via email prior to your appointment, at least 48 if possible. This provides the opportunity for me to review, verify insurance, makes the intake appointment more productive, and lastly verify that I am an appropriate counselor for you or your child. If you are unable to complete and return them before your appointment, no worries but please arrive 30 minutes early.
